You may print out the online form and mail it in if paying by check.


The conference format will be a hybrid of in-person and virtual. You can choose to attend in-person at the Omni Interlocken or virtually from your home or office.  Both types of attendees will have access to the live sessions, as well as the recorded sessions on-demand following the conference. 

**If you wish to change your registration type (i.e. In-Person to Virtual), please contact Jenny Patterson at jenny@societyhq.com.  The deadline to change your registration type is April 12, 2024.

NOTE: We will not accept registrations after the conclusion of the conference program on April 28, 2024. All attendees, whether viewing the sessions in-person or virtually, will have 60 days after the end of the live conference to complete the online evaluation and claim your CME credits. Questions?
