You may print out the online form and mail it in if paying by check.

Early Registration Rates extended through April 14, 2023.

The conference format will be a hybrid of in-person and virtual. You can choose to attend in-person at the Marriott Louisville Downown or virtually from your home or office.  Both types of attendees will have access to the live sessions, as well as the recorded sessions on-demand following the conference. 

**If you wish to change your registration type (i.e. In-Person to Virtual), please contact Jenny Patterson at jenny@societyhq.com.  The deadline to change your registration type is April 14, 2023.

NOTE: We will not accept registrations after the conclusion of the conference program on April 30, 2023. All attendees, whether viewing the sessions in-person or virtually, will have 60 days after the end of the live conference to complete the online evaluation and claim your CME credits. Questions?
